Privacy Policy

Privacy Statement and Data Protection

I am registered with the Information Commissioners Office (ICO) which means I need to tell you what data I am collecting from you and what I intend doing with it. This Privacy Policy lets you know how your personal information is being collected, stored, and shared and this is in line with EU General Data Protection Regulations.

 

For the purposes of the General Data Protection Regulations (GDPR) 2018, the data “controller” is  Alison. M. Speakman- Counselling and Supervision ICO Registration Reference Number: A8400421

Data I collect and why

 

The lawful basis of why I am collecting and processing your personal data is in order to fulfil my contractual obligations to you and to fulfil professional and legal responsibilities.

 

The data I collect is as follows:

 

Name: this is basic information and enables our ways of working.

 

Address, e-mail address and phone number: I use these as methods of contacting you regarding our supervision sessions. I will contact you via your preferred method of contact.

 

Personal Code Identifier: this code is assigned to your record and is allocated on commencement of therapy/supervision

 

Therapy/Supervision session notes: These notes are anonymised as far as practicable and are identified via the allocated personal code. Brief handwritten session notes are maintained of the key issues discussed. This supports our relationship and facilitates professional and ethical ways of working. A record of the session is a requirement of the insurance body.

 

Secure Storage

 

I make every effort to keep all information confidential. Your personal information is stored securely and are kept as follows:

 

Records.

All handwritten records which have personal identifiers are kept in a locked filing cabinet and separately from the session notes (which are anonymised as far as practicable).

 

Mobile Phone

The main use is to manage appointments. Your phone number will be kept in my business phone which is code protected.

 

E-mail

The main use is to manage appointments only. 

 

Diary

The main use is to organise sessions, a current paper diary is kept with first names and appointment details is maintained.

Right to be Forgotten- Data Destruction

 

You have the right to ask me to erase any information that I hold about you. This includes your personal information that is no longer relevant to original purposes, or if you wish to withdraw consent. In all cases and when considering such requests, these rights are obligatory unless it’s information that I have a legal obligation to retain.

My Insurance company dictates the timeframe in which I maintain your records and are as follows:

I will keep your name, your unique code and session notes for five years. Following which they will be shredded.

My diary will be destroyed 3 months following the diary year end.

I will delete your phone number from my mobile phone and your e-mail details (if stored electronically), three months after our work finishes.

 

Sharing your information

 

The information disclosed during the course of our supervision is confidential and it is highly unlikely that I will share your data. However, there are legal exceptions. For example, if a court order is received and a legal obligation arises.  Also, I have supervision on a monthly basis where I explore my work with a BACP Registered and Accredited Supervisor, your personal details are not disclosed.

 

Right to Access 

 

You have the right to ask for a copy of your personal information, free of charge. You also have the right to ask me to amend or change any incorrect information about you. As the supervisee, you have the right to receive your personal information, and also have the right to transfer that information to another party. 

 

If you wish to withdraw your consent to my use of your data at any point please let me know in writing by letter or email

 

Policy Review

 

This policy will be reviewed in response to service change and development.
